Notice on Application for Digital Health Declaration Form for Foreign Passengers (updated on September 6, 2021)

I. Starting from November 28th 2020, foreign passengers who are to fly to China from Australia need to apply for Digital Health Declaration Form with "HDC" mark online. Foreign passengers could access the Health Declaration Certificate system  by logging on the website of via computer/smart phone, or scan the QR code below.


After registering an account with your email address, please log in to fill in your information, declare your health conditions and upload the negative PCR nucleic acid test and IgM anti-body test results, the information page of your passport, itinerary, and visa page and all other required documents.

The Chinese Embassy or Consulate-General in Australia will review the documents and issue the Health Declaration Certificate via the same online system. Upon approval, a green QR code with an "HDC" mark will be received. Foreign passengers must present the code, and follow the procedures for inspection by relevant airlines during boarding.

The Chinese Consulate-General in Brisbane will only review the application of passengers who take the tests at institutions in Queensland.

II. Test Requirements

Passengers going to China via direct flights must take nucleic acid and IgM anti-body tests within 2 days before boarding. They must take the tests at qualified institutions certified by Australian government. For example, if the test is taken on 1st Dec, the flight should be no later than 3rd Dec.

III. Important Reminders

A. Passengers must take direct flights from Australia to China. The application of passengers taking connecting flights will be rejected.

B. Passengers who have completed vaccination (having had all the required doses of the vaccine), please upload a government-issued vaccine certificate. Please be advised that you need to take the pre-flight nucleic acid and IgM anti-body tests at least 14 days after the final dose. Otherwise, your application will be rejected.

C. Passengers who have a COVID-19 infection record and currently live in Queensland, please contact the Consulate-General ( at least 4 weeks before the flight to get specific instructions on the application process for the HDC code. Otherwise, your application will be rejected.

Passengers with  COVID-19 infection record refer to those who:

1) have been diagnosed with Covid-19 by a medical institution, or;

2) have never been diagnosed with Covid-19 by a medical institution, but any of the test results of PCR, IgM or IgG antibody is positive (not including positive test result due to vaccination) .
